We provide updates on progress and goals, and we welcome questions and input from families.

    Staff-to-Patient Ratio

    • The staff-to-patient ratio is an important consideration when choosing a rehab facility for seniors. Without a team that is properly staffed, residents may not receive the care they need and deserve.     • For 12 to 21 days after an operation, Derby Health and Rehab offers inpatient rehabilitation services. These services are designed to help patients regain their strength and independence. From pain management therapy to help with mobility, our nursing staff will work with you to create a care plan that meets your specific needs.

    Patient-Centered Approach

    • The rehabilitation team at Derby Health and Rehab takes a patient-centered approach to care. This means that we work with you and your family to create a care plan that fits your specific circumstance.

  • ADLs

    • The activities of daily living (ADLs) are the basic tasks that we all do to take care of ourselves. They include things like eating, bathing, dressing, and using the restroom. As we age, these tasks can become more difficult and challenging to do on our own. Tending to all of these responsibilities can be tough for an amateur taking on the challenge of caring for their parents. When a senior’s ADLs become a burden for the caregiver, the elderly’s standard of living begins to wane too.

    Safety

Falls can cause serious injuries like hip fractures and head injuries, and can even lead to death.     • The nursing staff at Derby Health and Rehabilitation is trained to help prevent falls and keep our residents safe. We take measures like installing grab bars in bathrooms and showers, providing mobility assistance, and conducting regular safety checks. We also have an active nursing staff so that someone is almost always available in case of an accident or injury. With our physical therapy services, we also provide rehab for seniors who sustain injuries.

    Medicaid

    Self-Funding

  • Does Your Loved One Need More Assistance Than You Can Provide?

Derby Health & Rehab can provide 24-hour assistance with ADLs to ensure that your loved one is always safe and comfortable.

    Is Your Home Set Up for a Disabled or Elderly Person?

    • If your home is not set up for a disabled or elderly person, it may be difficult for them to get around. This can lead to falls and other accidents. Long-term care facilities are designed specifically for seniors and people with disabilities. This means that all areas of the facility are accessible and safe for them to use.

    Do They Seem Isolated or Lonely When They Are at Home by Themselves?

    • If your loved one is spending most of their time alone, they may become isolated or lonely. This can have a negative impact on their mental and emotional health. In a long-term care facility, your loved one will be around other people who are in similar situations. This can help them to form new friendships and socialize more regularly.

    Have There Been Any Recent Falls or Accidents in the Home?

    • If your loved one has recently fallen or had an accident in the home, it may be time to consider a long-term care facility. Derby Health & Rehab can provide 24-hour assistance to help prevent falls and accidents, and ensure the health and safety of your loved one.

  • We accept traditional Medicare as well as most all Medicare Advantage plans such as:

    • United Healthcare

    • Humana

    • Aetna

    • Ascension

    • Blue Cross Blue Shield

    We also accept a wide variety of commercial insurances.

  • We accept Kansas Kancare Medicaid programs as well as most all Long Term Insurance policies and Private Pay

  • We encourage families to visit their loved ones whenever possible.  You are welcome to come visit 24/7, we only ask that you are respectful during sleeping hours.

  • ABSOLUTELY!  We love animals and feel it is great for heeling.  Your pet is welcome to visit along side your visitors as long as they are well behaved and up to date on their vaccines.

  • We have a wonderful activities director that puts together a great calendar of events that our residents love to take part in.

  • Our food services offers a set menu for lunch and dinner.  Breakfast is ala-cart.  We also have ala-cart menu options available in place of our set menu schedule if interested in a different meal. We are able to meet whatever food consistency you need.

  • We recommend packing comfortable clothes and pajamas for around 5 days.  We will do your laundry while you are with us unless otherwise discussed.  You will also need a pair of shoes to wear while working with therapy.   We encourage you to bring any items from home that would provide comfort for you during your stay.

    Please also bring insurance cards and photo ID.

  • Insurance allows for us to provide 40 minutes of physical therapy 5 times a week and 40 minutes of occupational therapy 5 times a week with our own in house therapy team.  We also provide speech therapy when indicated.

  • This is largely dependent on your insurance and what they will allow as well as your progress with therapy.  On average, the goal is 14-20 day stay.

    • If you desire to come for skilled nursing/short term Rehab, reach out to your hospital case manager and they can send us a referral packet.
